closing practice September 15, 2023
Dear Patients,
It is with very mixed feelings that I am writing to let you know I plan to close my clinic on September 15, 2023. I have accepted a position at the Texas A&M School of Nursing as Assistant Dean of the Round Rock campus. The new location and responsibilities that come with this role will prevent me from being able to continue taking care of your medical needs.
The last day I will see patients is September 15. At this time Michelle and I will only be making follow-up plans for refills and to transfer records. The Bryan/College Station area is fortunate to have an abundance of experienced and compassionate physicians, nurse practitioners, and physician’s assistants to take over your care. For your convenience, I have included a short list of names and contact information for a few local primary care providers along with a records release authorization form. some offices may not accept all insurance plans, so you are encouraged to contact the office directly or contact your insurance company’s provider directory for a comprehensive list of possible providers available to you.
Once you have identified a new provider, please complete the release of information form with their information and return to the office. additionally, your new provider may send a release of information form to us via secured fax (979-485-9524). all major appointment notes and diagnostic findings from the last two years will be forwarded to your new provider. due to the volume of requests, records may take 4-6 weeks to be transmitted. If you would like a copy of your complete chart please contact us by phone with your request and a complete copy of your records will be made for you at the cost allowed by law.
Lastly, some of you have known me from the very beginning of my journey in the healthcare field, and I’ve known many of you through pregnancies, babies that are now adults and have become parents themselves. Through times of health and sickness, happiness and grief, we have navigated through all of these times and you have trusted me to be a part of your life’s journey. I never have, nor will I ever take that responsibility and privilege for granted. Additionally, many of you have shared your love, energy, and prayers during transitions in my life, and I am eternally grateful for your continued support.
Change is never easy, but I am excited for the learning and challenges that will come with this new opportunity. Michelle and I wish you nothing but the best of health and many blessings for your future.
